All treatment records are protected under the federal regulations governing Confidentiality of Alcohol and Drug Abuse Patient Records, 42 Cod of Federal Regulations (CFR) Part 2, and the Health Insurance Portability and Accountability Act of 1996 (HIPAA), 45 CFR, Parts 160 and 164, and cannot be disclosed without written consent unless otherwise provided for in the regulations. A consent may be revoked at any time except to the extent that action has been taken in reliance on it, and that in any event the consent expires automatically.
Bhadra Recovery Company LLC will use and disclose your health information for treatment, to obtain payments from an insurance company, and for administrative purposes as permitted by HIPAA and applicable law. We are required by law to maintain the privacy of your health information and provide you a notice of our legal duties and privacy practices with respect to that information and to provide you with notice of a breach of your unsecured protected health information.
Your written authorization will be required to disclose your health information with the exception as noted above such as payment for treatment or as permitted by law. You can contact info@bhadrarecovery.com for a release of information or your specific treating clinician. You have a right to a copy of your medical records, including to request corrections. The intake documentation includes the policies on your individual rights and privacy.
